Effective Tuesday, March 17, the Lakeville Police Department lobby will be closed to the public. The Data Request form and Permit to Purchase application is available on this website.
Data requests for information about you and Permit to Purchase applications must be submitted with a copy of a valid state ID, driver’s license, military ID or passport. These can be submitted via email, PoliceRecords@lakevillemn.gov, or mail to: Police Department, 9237 183rd St. W., Lakeville, MN 55044.
Officers will not be dispatched to respond to data requests or permit to purchase requests.
